Unstable coronary artery disease, such as unstable angina and non-q-wave myocardial infarction:
Subcutaneous injection 120 IU/kg weight, twice daily. The maximum dose is 10000 IU/12 h. Treatment should last at least 6 days and may be extended if the doctor deems necessary. Thereafter, a fixed dose is recommended for prolonged treatment until a revascularization procedure (such as percutaneous intervention (PCI) or coronary artery bypass graft (CABG) is performed. Low doses of acetylsalicylic acid are recommended, unless contraindicated. The total treatment cycle should not exceed 45 days, and the dose should be selected according to the patient's gender and weight:
Women under 80kg and men under 70kg: 5000IU every 12 hours.
Women weighing more than 80kg (including 80kg) and men weighing more than 70kg (including 70kg) : 7500IU every 12 hours.
Prevention of surgically related thrombosis
Patients with moderate risk of thrombosis:
2500 IU is injected 1-2 hours before surgery and 2500 IU is injected every morning after surgery until the patient can move, which generally takes 5-7 days or longer.
Subcutaneous injection of 5000 IU is administered once a day for 12 to 14 days. Patients with persistent activity limitations may be longer. Anticoagulant function is not normally monitored.
Patients with a high risk of thrombosis (specific patients with certain tumors and certain orthopedic procedures) :
Preoperative subcutaneous injection of 5000 IU at night, postoperative subcutaneous injection of 5000 IU per night. Treatment should continue until the patient is active, generally taking 5-7 days or longer. Additionally, 2500 IU can be injected 1-2 hours before surgery and 8-12 hours after surgery. Then inject 5000 IU per day in the morning.
Treatment after total hip replacement should last up to 5 weeks, even if the patient is mobile.
Warning different LMWH has different characteristics at different recommended doses. So pay special attention and check the instructions for different products.
No intramuscular injection.
Heparin sodium is compatible with isotonic sodium chloride solution (9mg/ml) or isotonic glucose solution (50mg/ml) placed in glass and plastic bottles. The solution must be used within 12 hours.
